骨科醫(yī)學(xué)論文-閉合復(fù)位交鎖髓內(nèi)釘治療脛骨干骨折
【摘要】 目的 觀(guān)察閉合復(fù)位交鎖髓內(nèi)釘治療脛骨干骨折的療效。方法 回顧分析采用閉合復(fù)位交鎖髓內(nèi)釘內(nèi)固定治療的脛骨干骨折患者32例。結(jié)果 所有患者獲得平均12個(gè)月( 5～24個(gè)月)隨訪(fǎng), 均達(dá)到骨性愈合,平均臨床愈合時(shí)間4.8個(gè)月。其中骨折延遲愈合4例，采用動(dòng)力化后骨折愈合；發(fā)生骨不連2例,行二次手術(shù)植骨達(dá)骨性愈合。所有患者無(wú)血管神經(jīng)損傷,未發(fā)生感染。療效根據(jù)JohnerWruhs評(píng)定:優(yōu)24例,良5例,可2例,差1 例,優(yōu)良率90.6%。結(jié)論 閉合復(fù)位交鎖髓內(nèi)釘治療脛骨干骨折損傷小、固定牢靠、療效肯定,對(duì)于骨折延遲愈合患者動(dòng)力化可加快愈合。

Therapeutic effect of closed reduction and interlocking intramedullary nails on fracture of shaft of tibia DENG Songbo, ZHANG Yaoqiang, XU Huorong. Huidong County People’s Hospital, Guangdong, Huidong 516300, China 【Abstract】 Objective To observe the therapeutic effect of closed reduction and interlocking intramedullary nails on fracture of shaft of tibia. Methods the data of 32 patients with fracture of shaft of tibia treated by closed reduction and interlocking intramedullary nails were retrospectively analyzed.Results All the cases were followed up for average 12 months (5～24 months), and all the patients got bone union. The mean clinical union time was 4.8 months in which 4 cases got bone fracture delayed union. After using postmotivation fracture union,2 cases was not healed, after the second operation with bone graft, the 2 cases got bone union. None of case occurred nervi vasorum injury or infection. According to the JohnerWruhs function scale,24 cases were excellent, 5 cases were good, 2 cases were acceptable and 1 case was bad, and excellent and good rates reached 90.6% .Conclusion The application of closed reduction and interlocking intramedullary nails in the treatment of fracture of shaft of tibia has advantages of little injury, stable fixation, definite therapeutic effect. 交鎖髓內(nèi)釘治療脛骨干骨折近年來(lái)開(kāi)展較多，具有可解剖復(fù)位、固定牢固的優(yōu)點(diǎn)。是目前治療脛骨干骨折的首選方法。本文選取自2003年3月至2008年3月我院采用閉合復(fù)位交鎖髓內(nèi)釘內(nèi)固定治療的脛骨干骨折患者32例進(jìn)行回顧分析，臨床效果滿(mǎn)意，報(bào)告如下。
