【摘要】 目的 研究雷諾嗪對(duì)兔心房肌細(xì)胞快鈉電流的影響及其是否存在使用依賴性阻滯作用。方法 應(yīng)用全細(xì)胞膜片嵌記錄方法,了解雷諾嗪對(duì)兔單個(gè)心房肌細(xì)胞快鈉電流的作用,及在不同刺激頻率(1Hz、3.3Hz、5Hz)時(shí)對(duì)快鈉電流的影響。結(jié)果 30μmmol/L雷諾嗪對(duì)兔心房肌細(xì)胞快鈉電流有明顯的抑制作用,其IC50為(25.6±1.8)μmmol/L,并且隨著刺激頻率的增加其作用加強(qiáng),存在使用依賴性。結(jié)論 雷諾嗪對(duì)兔心房肌細(xì)胞快鈉電流有一定的抑制作用,其存在使用依賴性阻滯作用。

Methods Standard whole cell patch clamp technique was used to study the effect of ranolazine on the fast sodium channel current and the use-dependent blockade caused with different frequencies (1Hz, 3.3Hz and 5Hz) to stimulate the cells. Results The 30μmol/L ranolazine significantly reduced INa with an IC50 value of (25.6±1.8)μmmol/L and produced a frequency-dependent inhibitory effect on INa with obvious use-dependence. Conclusion Ranolazine can inhibit the fast sodium channel current in rabbit atrial myocytes and indeed has a use-dependent effect.

雷諾嗪(Ranolazine)是一種新型代謝調(diào)節(jié)劑,多個(gè)臨床試驗(yàn)證實(shí)其有確實(shí)的抗心絞痛作用,且不影響血壓和心率,于2006年1月27日正式被美國(guó)食品與藥品管理局批準(zhǔn)用于心絞痛的輔助治療[1-2];A(chǔ)研究發(fā)現(xiàn),雷諾嗪有潛在的抗心律失常作用,作用上類似于胺碘酮。其治療心絞痛的作用可能主要通過(guò)抑制心肌細(xì)胞的晚鈉電流(INa-L),減輕細(xì)胞內(nèi)鈣超載而實(shí)現(xiàn)的[2]。目前關(guān)于其對(duì)心臟快鈉電流(INa)的作用尚存在爭(zhēng)議[2-4]。本實(shí)驗(yàn)擬應(yīng)用全細(xì)胞膜片嵌記錄方法,了解雷諾嗪對(duì)兔心房肌細(xì)胞INa的影響及其是否存在使用依賴性阻滯(use-dependent)作用醫(yī).學(xué)全.在.線m.payment-defi.com。
1 材料與方法
1.1 心房肌細(xì)胞的分離
新西蘭純種大耳白兔6只,雌雄不拘,體重1.5~2kg。采取鹽酸氯胺酮(Ketamine)120~160mg/kg體重耳靜脈注射麻醉,成功后迅速開(kāi)胸切取心臟,置于冰冷的無(wú)鈣臺(tái)氏液中沖洗,剪去心包,心臟經(jīng)修飾后連接在Langendorff心臟灌流裝置上;經(jīng)主動(dòng)脈根部逆行用氧飽和的無(wú)鈣臺(tái)氏液灌流(20mL/min,恒溫37℃)10min,再用含0.4g/L膠原酶I(Sigma公司)、2g/L小牛血清蛋白(Sigma公司)的無(wú)鈣臺(tái)氏液反復(fù)灌流15~20min,至心臟膨大、松弛后從灌流裝置上取下;去除心室肌,加數(shù)滴含酶無(wú)鈣臺(tái)氏液后將心房肌剪碎成1mm3的組織塊,加入含膠原酶I和小牛血清蛋白的無(wú)鈣臺(tái)氏液溫孵20min;最后過(guò)濾離心保存在KB液中備用。
