【摘要】 目的 探討β�舶肴樘擒战Y(jié)合蛋白��3(Galectin��3)表達(dá)與肺腺癌淋巴及胸膜轉(zhuǎn)移的關(guān)系。方法 采用鼠抗人Galectin��3單克隆抗體和免疫組化鏈霉素抗生物素蛋白-過(guò)氧化物酶(SP)法，檢測(cè)63例肺原發(fā)性腺癌和34例胸膜轉(zhuǎn)移性腺癌(均來(lái)自肺，影像學(xué)顯示為非直接侵襲病灶)中Galectin��3的表達(dá)。結(jié)果 發(fā)現(xiàn)Galectin��3同時(shí)存在于胞漿及胞核，未見(jiàn)單獨(dú)的胞核表達(dá)。肺原發(fā)性腺癌胞漿、胞核表達(dá)陽(yáng)性率分別為92.1%、31.7%，胞核表達(dá)與淋巴結(jié)轉(zhuǎn)移(P=0.03)、臨床分期有關(guān)(P<0.05)。胸膜轉(zhuǎn)移性腺癌胞漿、胞核陽(yáng)性率分別為97.1%、85.3%，胞核表達(dá)明顯高于肺原發(fā)性腺癌，差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論 細(xì)胞核內(nèi)Galectin��3可能增強(qiáng)肺腺癌細(xì)胞侵襲轉(zhuǎn)移潛能，可作為判斷淋巴結(jié)轉(zhuǎn)移和/或胸膜轉(zhuǎn)移的生物學(xué)指標(biāo)。

To investigate the correlation between galectin��3 and lymphatic metastasis and pleural metastasis in adenocarcinoma of the lung. Methods Immunohistochemistry was used to determine expression of galectin��3 in 63 primary lung adenocarcinomas specimens and 34 adenocarcinomas metastatic of pleural specimens. Results The patterns of galectin��3 expression were cytoplasmic and nuclear. All cases of nuclear galectin��3 also showed a cytoplasmic immunoreactivity. In primary tumors the positive galectin��3 expression was found in 92.1% of the cytoplasm and 31.7% of the nucleus. Only the nuclear location of galectin��3 was associated with lymphatic metastasis　(P=0.03)　and clinical stages(TNM stages)(P<0.05). In pleural secondary adenocarcinomas the positive expression rate of galectin��3 in the cytoplasm and nucleus was 97.1% and 85.3% respectively, and the nuclear staining was significantly higher than the primary tumour(P<0.05). Conclusion Nuclear expression of galectin ��3 can enhance the invasive and metastatic potential of lung adenocarcinomas and can be used as a biological marker of lymph node and/or pleural metastasis.

Key words： Beta�瞘alactosidase; Carcinoma, non�瞫mall cell lung; Neoplasm metastasis 在肺癌的研究中發(fā)現(xiàn)β�舶肴樘擒战Y(jié)合蛋白��3(Galectin��3)多呈高表達(dá)，但有關(guān)其與肺腺癌淋巴轉(zhuǎn)移及胸膜轉(zhuǎn)移的關(guān)系研究報(bào)道較少。本研究旨在應(yīng)用免疫組織化學(xué)SP染色方法來(lái)檢測(cè)肺原發(fā)性腺癌及胸膜轉(zhuǎn)移性腺癌中Galectin��3的表達(dá)，初步探討Galectin��3與肺腺癌淋巴結(jié)轉(zhuǎn)移及胸膜轉(zhuǎn)移的關(guān)系。

1 資料與方法

1.1 資料

1.1.1 臨床資料 選取2006年至2007年山東省立醫(yī)院呼吸內(nèi)科因胸腔積液原因待查住院患者行內(nèi)科胸腔鏡活檢確診為胸膜轉(zhuǎn)移性腺癌的石蠟標(biāo)本34例，(來(lái)自肺，影像學(xué)顯示為非直接侵襲病灶)，男15例，女19例，42～64歲，其中影像學(xué)或體格檢查發(fā)現(xiàn)伴淋巴結(jié)轉(zhuǎn)移(腫大)21例;另有2007年胸外科手術(shù)切除的肺腺癌石蠟標(biāo)本63例，男29例，女34例，45～74歲，其中伴有淋巴結(jié)轉(zhuǎn)移33例。所有病例取標(biāo)本前未行任何放化療，均有病例診斷結(jié)果。

1.1.2 試劑 實(shí)驗(yàn)所用Galectin��3鼠抗人單克隆抗體、SP試劑盒(含二抗、三抗工作液)及DAB顯示劑均購(gòu)于福州邁新公司;蘇木素購(gòu)于北京中衫金橋生物公司。
